The Transfolio Fragility Index
A health score that explains itself.
RAG status tells you how someone feels about a programme. The Fragility Index tells you where it is structurally weak, scored the same way across every initiative, every scan.
3
independent dimensions
15
governance themes per scan
5
lifecycle stages, each with its own questions
5 min
for a driver to complete a scan
What it measures
Three dimensions, five governance themes each. Every scan asks one question per theme.

- Financial integrity
Leadership & Decision Effectiveness
Are the right people deciding at the right speed?
- Decision authority
- Risk handling discipline
- Escalation effectiveness
- Governance effectiveness
- Decision stability
Delivery & Dependency Control
Can the team actually deliver it?
- Dependency control
- Integration control
- Capacity & resource health
- Issue resolution health
- Reporting integrity
The right questions for the stage you're in
A programme seeking investment and one going live fail in different ways. The questions change with the lifecycle stage, so the score stays meaningful from mandate to adoption.
- 1 · Investment & Mandate
- 2 · Design
- 3 · Build & Validate
- 4 · Deploy
- 5 · Adopt & Stabilise
Four answers, no fence-sitting
Each question is answered on the same four-point scale. There is no middle option to hide in.
From score to decision
Published rules, not opinions.
Every initiative lands in one of three classifications by the same transparent rules, so the steering committee debates the response, not the rating.
INTERVENTION NEEDED
- Overall score below 35, or
- any single dimension below 33%, or
- a drop of 15 points or more since the last scan
DECISION NEEDED
- Overall score below 75, or
- Leadership & Decision Effectiveness below 50%, or
- a drop of 10 points or more since the last scan
ON TRACK
- None of the above
- Still watched for drift on every scan
Why three dimensions matter
An average can hide a collapse.
An initiative scoring 44 overall sounds like "needs attention". Look at the dimensions and delivery has collapsed to zero. That's why any dimension below 33% triggers intervention on its own, and why the Executive View marks it with a !.
Yes, it's self-assessed. Here's why it still works.
Specific, not sentiment
Drivers rate concrete governance conditions, not how they feel about the programme.
Comparable everywhere
Every initiative answers the same structure, so one optimistic team stands out.
Trends are hard to spin
Each scan is compared with the last. Drift shows even when a single scan looks fine.
Contradictions get flagged
The Executive Briefing calls out where a driver's notes don't match their scores.
